Level I could include assistance with showers, laundry,
lab coordination and miscellaneous assistance.
Level II could include medication
administration (set-up by licensed personnel and reminders
by PCAs) and laundry or consistent AM and PM Care; lab
coordination.
Level III could include medication
administration by PCAs, showers, laundry, miscellaneous
assistance and consistent either AM or PM Care and lab
coordination.
Level IV could include medication
administration by licensed personnel, showers, laundry,
miscellaneous assistance and lab coordination.
Level V could include medication
administration by licensed personnel, showers, laundry, lab
coordination and either AM or PM Care.
Level VI includes all nursing
services.
Out-of-pocket supplies (such as briefs,
gloves, wipes, etc.) are billed in addition to the Level of
Care. There will be no pro-ration to the monthly Level of
Care fee unless a Resident is absent from the facility for
more than 30 days.
The nursing department will determine a
Resident’s Level of Care. A Resident’s Level of Care may
increase or decrease. Any increase to a Resident’s Level of
Care will be in consultation with the Resident or his agent
and will only occur after at least 15 days prior written
notice.
